Preparing for your first appointment
At a chiropractic examination, the first thing your chiropractor will do is to examine you and have a personal discussion with you. This will conduct a thorough medical history and review medical, X-ray or MRI records you’ve brought with you etc.
To simplify the process, you have the opportunity to download our data protection and treatment information form. You can bring these with you to your first appointment, already filled out and signed.
Your information will of course be treated confidentially and is subject to medical confidentiality.
Sending us your documents
You have the option to send us your filled out signed data protection and treatment information form in advance.
Online-Editing
