Preparing for your first appointment

At a chiropractic examination, the first thing your chiropractor will do is to examine you and have a personal discussion with you. This will conduct a thorough medical history and review medical, X-ray or MRI records you’ve brought with you etc.

In order to give us a better overview of your complaints and your initial situation, as well as to simplify the process, you have the opportunity to download our medical history form as well as our data protection and treatment information form. You can bring these with you to your first appointment, already filled out and signed.

Please take sufficient time to carefully go through and complete our medical history form. It is an important basis for your first treatment in our practice.

If you do not wish to provide certain personal details in writing, you are free to do so. Your information will of course be treated confidentially and is subject to medical confidentiality.
